If you are directing your donation for a patient, please bring with you the patient's name, their date of birth and/or their medical record number and provide at the time of registration. If you need assistance obtaining this information please contact the donor center at (626) 218-7171.
Female donors, if you have ever been pregnant you must contact the blood donor center before scheduling a platelet appointment.
