Description
Patty Barnett Mouton has served as Vice President for Outreach & Advocacy at Alzheimer?s Orange County since 2005. She manages community and clinical outreach, as well as the Advocacy and Public Policy activities at the local, state and federal levels. She has served as co-chair of the OC POLST Coalition, now known as the OC Advance Care Planning Partners, since 2010. She currently serves in leadership roles on both the CalOptima OneCare Connect and Member Advisory Committees, and will soon also begin serving on the Provider Advisory Committee. Active in the work of the OC Aging Services Collaborative and the OC Strategic Plan for Aging, Patty serves as chair of the Social Isolation pillar for this project. Patty is active on theleadership council for the Roman Catholic Diocese of Orange Whole Person Care Initiative, which is providing robust community education about improving access to excellent end of life care. She serves on the Board of Directors for the Sacred Dying Foundation and the Health & Nutrition Committee for SCAC in Orange County.
Patty has been a featured lecturer at many national meetings including: National POLST Paradigm Conference, California Council of Geriatrics and Gerontology, American Society on Aging National Conference and the California Association of Health Services at Home
Conference. She has had several articles published about the work done with Dr. Vincent Nguyen in the area of Advance Care Planning. She holds graduate certificates in Fundamentals of
Gerontology from USC and in Palliative Care Chaplaincy from CSU San Marcos, as well as a BS in Human Services from Springfield College.
